For us at Personal Care Hospice Services, Inc. hospice does not hasten the loss of life but rather it enriches the remaining days of the individual as the disease takes its natural course.

Types of Care and Services Provided

ROUTINE HOME CARE
We provide residential care in the patients’ homes and establish partnerships with long-term care facilities and nursing homes, assisted living facilities and residential care facilities for the elderly.

CONTINUOUS CARE (Intensive Comfort Care SM)
If a patient develops the need for more constant care, we can provide shifts of hospice staff at the home for a brief period so the patient can avoid being transferred to a hospital.

DEDICATED INPATIENT UNITS
If a patient needs care beyond what can be provided at home, we have special arrangements with local hospitals and other facilities where patients may receive necessary and appropriate care and return home as soon as possible

RESPITE CARE
Limited to five consecutive days, respite care provides a brief “respite” for the patient’s primary caregiver by admitting the home care patient to an institutional setting without meeting the “inpatient” pain and symptom management criteria.

24 HOUR / 7 DAYS A WEEK PHARMACY
Any patient who enters hospice must be evaluated at the time of admission to the hospice program, and if there is a dietary need, a dietitian should be provided to assist and educate the patient and family. Common problems such as nausea and vomiting ore controlled with medications in some cases, while for others, a change of diet is sufficient. Hospice patients and families need to be aware of the changes occurring in the patient’s body. For adverse symptoms, there is a corresponding medication or pharmacy-related intervention that can be offered to the patient.

Drugs, Equipment and Medical Supplies
Personal Care Hospice Services, Inc. covers all prescription drugs, over-the-counter medications, medical equipment and supplies required to treat life-limiting illnesses related to the admission diagnosis.

Telecare
A nurse is just a phone call away 24/7, all year round. Our staff will also visit the patient at home whenever necessary to respond to crises related to the illness.

Bereavement Support
We offer emotional support for family members as they come to terms with the potential loss or the recent passing of their loved one.

Hospice Criteria Eligibility

Requirements for Hospice will be determined when a patient meets the following Criteria:

  • The Patient has a chronic illness such as:
    • Alzheimer’s
    • ALS
    • Cancer
    • Cardiac Disease
    • Debility
    • Dementia
    • HIV/AIDS
    • Liver Disease
    • Pulmonary Disease
    • Renal Disease
    • Stroke
  • A terminal illness with a limited life expectancy of 6 months or less if the conditions take its natural course.
  • The focus will be on palliative treatment, including pain control and emotional/spiritual support.
  • The patient’s physician and hospice physician authorizes hospice care.
