Continuous Care

If a patient develops the need for more constant care, we can provide shifts of hospice staff at the home for a brief period so the patient can avoid being transferred to a hospital.

With Continuous Care, the client is able to access services such as:

• Safety Supervision
• Transportation Arrangements
• Help with Bathing and Showering
• AM and PM Care
• Incontinence Care
• Assistance with Mobility
